Sam (Jared Padalecki) and Dean (Jensen Ackles) investigate several mysterious deaths of men who appear to have died of fright. The Winchester brothers visit one of the victims in the morgue, where Dean becomes infected with the same illness: an illness that starts as a bout of anxiety, before turning into unimaginable terror and stopping the heart of its victim.
